Researchers create digital health platform to improve remote monitoring for stroke patients

Researchers at Wake Forest University School of Medicine have created a digital platform targeted at improving remote monitoring for stroke patients while also enhancing care coordination. This digital health platform, named COMprehensive Post-Acute Stroke Services – Care Plan (COMPASS-CP), will help improve outcomes for at-risk patients by employing algorithms for a personalised treatment by considering social determinants of health and clinical data attained from Electronic Health Records (EHRs). Through the integration of technologies, this platform will enable virtual care, especially remote monitoring.
